This tree crashed into a house in the Thornridge section of Falls Township during Hurricane Sandy.

The Falls Township Code Enforcement Department has released a form to help homeowners dealing with damage caused by Sandy.

The form asks residents to mark where the damage to their house occurred. It also asks for a written description of the damage.

The application form was approved by the Board of Supervisors and must be submitted to Code Enforcement.
